Making an Appointment
Please have the following information ready when calling to schedule your appointment:
  • Patient’s full name
  • Date of birth
  • Contact number
  • Insurance information
  • Purpose of the appointment
It is very important that prior to your appointment, you verify that we have the correct insurance information on file. Failure to inform us of an insurance change may lead to unpaid charges that will be the patients’ responsibility.

Patient Information

New Patient Forms
  • To save time you may download the new patient forms in the Patient Forms section and fill them out in advance, or arrive at least 25 minutes early to complete the necessary paperwork.
Transferring Records
  • Medical records can be transferred to West Marion Family Practice by completing our authorization form. You can use the Medical Records Release form found in the Patient Forms section. Please fill out form and bring it to our office.

What to bring to your Appointment
  • Please bring your ID and your insurance card to each visit and notify us of any changes in your address, phone number, and/or insurance information. For new patients, please bring a list of your medications or prescription bottles.
Billing and Insurance
Payment Policies
  • We accept cash, personal checks, VISA, Mastercard, Discover, and Amex.
  • All balances are due within thirty (30) days of the initial statement date. Credit card payments can be made in person, telephone or online.
  • Our practice is contracted with most major insurance companies in the area. Please contact our office to verify if we are contracted with your plan.
